By completing the below form, I attest that I have watched the required MnTC x MiMS training video. (password 1%E!xeY%). I also agree to uphold the following as a Team TC South or Team Challenge Mentor:

• Keeping all member’s names, identifiable information, and personal information confidential. This is especially important because they are in a recovery program, so it isn’t appropriate to share with others how you know them other than they are in the MiMS program. Members should only disclose their status in recovery programming on their own accord; please do not share names or personal information outside of MiMS.  

• In order to respect confidentiality, Mentors are not allowed to share photos/pictures or identifying information of Members on social media.

• In alignment with MiMS policies and MnTC program rules, volunteers may not get together with MnTC residents outside of MiMS-sanctioned activities or events.
